Maybe the pilot had a seizure or passed out while holding the throttles. His approach looked good until he accelerated. The point where he accelerated he should have been going into reverse to make a complete stop.
I crashed into the dock once. I was making my approach to the dock when I shifted to reverse to stop my engine died and I slammed into the dock.

Probably stepped on the gas instead of the brakes.
